perhaps Ryan's first experience as a slave kind of parallels the dream he had and it's the same in some ways but so, so different. So whereas Brendon 'smiled gently' in the dream, whoever's using him kind of smirks cruelly. just to show the two different branches of sex and how Ryan is trapped on the bad side. kind of thing.
